PARTICIPANT AGREEMENT/CONSENT FORM

Emerging Leaders Community Initiative (ELCI) is convinced that we need a fresh look at leadership development. This new approach will look at developing leaders at grassroot level from communities; it is a bottom – top approach to leadership development.
ELCI is a non-profit organization that seeks to identify, recruit, train and nurture leaders from grassroots.
At ELCI, we believe that nurturing and supporting grassroots leaders is central to sustaining our civility, democracy and to encouraging healthy, vibrant communities throughout Nigeria.
The training is for 6 days broken down into sessions and it is residential. I in consideration of being admitted as a participant of LEADERS IN TRAINING (after referred to as LIT), organized by EMERGING LEADERS COMMUNITY INITIATIVE (ELCI) This agreement is a precondition to my participation in the program and covers the entire term of my participation.
I hereby acknowledge, undertake and agree as follows:

• I affirm that to the best of my knowledge, I am fit and medically certified to be of good health to participate in the program, provided that full disclosure of contagious ailments shall be made to ELCI Management prior to registration.
• Sufficient notice of any special needs or dietary requirements must be disclosed to ELCI prior to registration.
• The program is a fully residential training program with a combination of classroom training and practical sessions. As such participants shall remain on the premises for the duration of the program. This may be waived by ELCI management upon sufficient notice, under special circumstances.
• Participants may be required to share accommodations (although exclusive accommodations may be provided upon request).
• Accommodation, toiletries, feeding and training materials shall be provided for the participants during the training period. Participants are however responsible for personal expenses such as travel, medicals, etc.
• ELCI reserves the right to admit, reject and/or discontinue training for any participant at any time during the training program for matters arising but not limited to misconduct, lack of commitment etc. where the participant is found to be dishonest, fraudulent, or engaged in any criminal act.
• I have not entered nor will I enter into any contractual obligations that are, or could reasonably be in conflict with this agreement, or any such obligations for the duration of this training.
• I shall participate in physical activities such as sports and other forms of exercises during the training.
• I consent to my pictures being taken during the duration of the program and waive the right to inspect or approve the use of the images or recordings (for educational and promotional purposes), the right to compensation, royalties or remuneration arising from or related to the use of the same.
• I understand that where a participant drops out or is unable to continue or falls short of the minimum attendance/participation requirement during the program, such participant may be required to forfeit the session and may not be allowed to re enroll with the next session as determined by ELCI.
• I agree to partner with other participants, ELCI, and its affiliates in ensuring the safety and security of personal lives and property.
• I hereby agree to indemnify ELCI and its affiliates, associates, staff, agents from and against all claims, demands, actions, suits, damages, liabilities, losses, settlements, judgments, cost and expenses whether or not involving a third-party claim which may arise out of or relate to any act or omission by me.
• ELCI Management reserves the right to seek redress for any breach, loss or damage caused as a result of non-adherence to the terms herein contained.
• I agree to take all necessary precautions to ensure the health and safety of myself and others.
• ELCI reserves the right to alter, modify, and make changes to this agreement as may become necessary.
• I shall conduct myself in a disciplined and godly manner in accordance with the expectations and ethical matters that are consistent with the core values of ELCI.
